Root Cause Analysis (RCA)
RCA or Practitioners (2 day)
This course examines many of the common pitfalls used in problem solving and how to avoid them. Building on that knowledge, students learn how to clearly define any event-based problem and how to use the Apollo four-step process to develop a causal understanding of that event and identify effective solutions. Students learn how to use RealityCharting software to build cause and effect charts, prepare RCA reports and effectively communicate RCA findings. The course also covers the principles of gathering evidence, interviewing and facilitating an Apollo RCA with a team of participants. RCA skills and knowledge are reinforced with many “hands-on” exercises that allow students to apply the Apollo RCA method to real world problems. Upon successful completion of this course, students will have gained the skill set of an Apollo RCA Practitioner.

The course will provide students with the knowledge necessary to effectively lead and implement an Apollo RCA program, as well as to, develop highly skilled facilitators. Upon successful completion of this course the student will have gained the skill set of an Apollo RCA Super User. With additional consulting and development a Super User can become the Apollo RCA Program Champion.
About Apollo Associated Services and RCA
The Apollo RCA method was a direct response to the challenges faced by industry in the early 1980s – high incident rates, complex problems, and significant consequences. Aiming to provide industry with a simply applied, yet effective, problem-solving tool, the Apollo method was introduced in the mid-1980s by Dean Gano, author of “Apollo Root Cause Analysis – Effective Solutions to Everyday Problems Every Time.” Each year since then, thousands of students worldwide have participated in Apollo training.
